Aging & Disability Resource Connections Information for Organizations, Agencies, Providers
The purpose of the Inclusion/Exclusion policy is to clearly define the types of entities, i.e., providers, agencies, organizations, and resources that are included/excluded for listing in the Aging and Disability Resource Connections (ADRC) Resource Directory.
Please read the policy before submitting your information.
By completing the Request for Inclusion form, you will be submitting information on the services your agency provides for consideration to be included in the Resource Directory.
Please note the following before submitting your information.
If you operate multiple sites or provide multiple services, complete a separate form for each site and service (program) that will be listed (separately) in the ADRC Resource Directory. Make duplicates of the form, as necessary.
Complete the form in its entirety with accurate information.
Be as detailed as possible in the “Description of Services" section.
