Behavioral Health Voucher Program for Child Care Providers

The Behavioral Health Voucher Program (BHVP) for Child Care Providers (CCP) through 605 Strong was designed to serve employees of South Dakota licensed and registered child care providers with mental health services. There are 17 clinics participating in the program. The program runs from August 1, 2023, through September 30, 2024 or sooner if all funding has been successfully allocated.

Initial Appointment/Assessment Information
The forms below are downloadable and required for BHVP CCP clients. The forms specifically comply with the BHVP CCP and its funding source. Electronic versions of these forms are allowed.

Data Collection
Data Collection is required at initial appointment/intake and discharge.

Receipt and Invoice Information
