This form is used by a parent, caretaker relative with legal guardianship/custody, or an attorney who is representing a child who would like to apply for location only services from the Division of Child Support.
Attach the appropriate application fee. (A $20 application fee is required for this service.)
In child custody or parental kidnapping cases, an application fee of $50 ($54 if no Social Security account number is furnished) is required. The application fee is waived for services requested by the courts.)
