Foster Care / Adoption / Kinship Care
This form is completed by a Family Services Specialist who contacts a reference for an individual who is applying to be licensed as a foster parent or approved as an adoptive parent or a family member who is interested in providing care for a related child.
This form may also be sent to a reference or the reference may be directed to obtain the form from the Internet, with a request that it be completed and returned to the Family Services Specialist.
Information provided on the form is used to assess the individual's character and ability to provide appropriate care for children.
