Assisted living centers, nursing homes, adult foster care homes or other individuals who know a person living within one of these settings are asked to complete the Initial and Continuing Need for Care Report when the person applies for the state's Medical Assistance Program.
This form is used to gather basic information about the person in need of assistance in order to determine whether the person meets the criteria for the Medical Assistance Program.
