A Grant from the Administration on Aging and the Centers for Medicare and Medicaid Services provided an opportunity for the Division of Adult Services and Aging to launch the state’s first Aging and Disability Resource Connections (ADRC) in the community of Sioux Falls.
ADRCs provide objective information, assistance and access to long term services and support options for our consumers.
ADRCs in South Dakota are called Aging and Disability Resource Connections and enhance access to available public and private services to help individuals live as independently as possible within their local communities.
Part of implementing ADRCs is the development of an online, comprehensive and user-friendly database consisting of long term services and support resources available in South Dakota. This online database will be available to ADRC staff as they provide information and resources to South Dakotans over age 60 and to adults over age 18 with physical disabilities.
The purpose of the inclusion/exclusion policy is to clearly define the types of entities, i.e., agencies, organizations, providers and
resources that are eligible for listing on the South Dakota Aging and Disability Resource Connections (ADRC) information and resource online database.

By completing the Request for Inclusion form, you will be submitting information on the services your agency provides for consideration to be included in to the online resource database. Please note the following before submitting your information.
The Request for Inclusion form submitted will be reviewed and a decision to include or exclude your agency will be emailed to you within 30 days from receipt of the form.
