The Medicaid Cost Report is to be filled out once a year by long-term care facilities who participate in the Medicaid Program. The purpose of the report is to obtain the patient-related costs incurred by long-term care facilities providing services according to all applicable state and federal laws, regulations and quality and safety standards. The report is used to establish reimbursement rates for the long-term care facilities.
