Food Stamp Child Care Expense Billing Verification

Food stamp recipients must have their daycare providers complete this form to verify daycare expenses.

The form must be signed by the daycare provider and the food stamp recipient to verify hours of care provided and the amount billed and paid. The form must be completed monthly.

• Once you have completed this form, submit it to your local Department of Social Services office.
