Track Application

Providers can click here to track their submitted provider enrollment applications.

To track an enrollment application, a provider must enter his/her Enrollment Application ID # and SSN/FEIN.

If you do not have your enrollment Application ID, you will need to contact South Dakota Medical Assistance (SDMA) at (605) 773-3495 or wait for your notification letter in the mail.
