TANF Child Support Enforcement Referral

This form is to be used by people applying for or receiving TANF. The form is used to help identify or locate an absent parent and obtain child support payments or resources legally due to the applicant or recipient. This form is to be completed at the time of application or when a new household member is added who has a parent absent from the home.
