BabyReady Program

The BabyReady Program is intended to improve maternal health outcomes through enhanced care coordination, services being provided in accordance with standards of care, and reducing barriers to care. Participating providers are eligible for enhanced reimbursement for care coordination and meeting prenatal care and postpartum care program objectives.

Program Objectives

Increase utilization of prenatal and postpartum care. Provide and connect pregnant and postpartum women with supports. Improve health outcomes for pregnant women and their babies.

Barriers to Care Initiative Report Form
Providers must select and implement one of the initiatives as outlined in the Barriers to Care Initiative form as a condition of participation in the BabyReady program. The Barrier to Care Initiative selected by the clinic should address a specific challenge the clinic has encountered as a barrier for recipients. The clinic must complete and submit a Barrier to Care Initiative annual reporting form each year by June 30. The report form serves as a guide for documenting outcomes, data, and the steps the clinic took to improve access to care.

Each enrolling clinic/practice location must complete an application and barriers to care initiative form. An addendum must be completed and signed by each individual provider listed on the application. An application is considered incomplete and will not be accepted if it does not include signed addendums for all providers listed in the application.

Please submit all forms to cmforms@state.sd.us or by fax to 605.773.5246.

Use the BabyReady Opt-In & Selection form to opt a recipient into the BabyReady Program and designate their provider.

If you have a recipient on your caseload that has miscarried, informs you they have selected another provider, or you are unable to establish care, the provider should complete a BabyReady Exit form.

Eligible Providers

Participating providers must be enrolled in South Dakota Medicaid. The following provider types are eligible to enroll in the program:

  • Physicians
  • Physician Assistants
  • Certified Nurse Practitioners
  • Certified Nurse Midwives
  • Rural Health Clinics
  • Federally Qualified Health Centers
  • Indian Health Services (IHS) and Tribal 638 Providers
