Pregnancy Program

The Pregnancy Program is intended to improve maternal health outcomes through enhanced care coordination, services being provided in accordance with standards of care, and reducing barriers to care. Participating providers are eligible for enhanced reimbursement for care coordination and meeting prenatal care and postpartum care program objectives.

Each enrolling clinic/practice location must complete an application and barriers to care initiative form. An addendum must be completed and signed by each individual provider listed on the application.

Eligible Providers

Participating providers must be enrolled in South Dakota Medicaid. The following provider types are eligible to enroll in the program:

  • Physicians
  • Physician Assistants
  • Certified Nurse Practitioners
  • Certified Nurse Midwives
  • Rural Health Clinics
  • Federally Qualified Health Centers
  • Indian Health Services (IHS) and Tribal 638 Providers
