Medicaid Provider Procedure Code Look-Up Tool

The Procedure Code Look-up Tool can be used by providers to identify circumstances when a procedure code is payable by South Dakota Medicaid for one or more provider types as well as the maximum allowable reimbursement rate. Providers should review all fields to determine if a code is payable and bill Medicaid for services at the usual and customary charge. The status indicated on the tool is not a guarantee of payment. Claims may deny for other reasons such as system edits or failure to meet South Dakota Medicaid coverage criteria. Providers should refer to the provider manuals for detailed coverage information and limitations and provider fee schedules to help determine if a specific code is covered for their provider type.

A description of each field is provided below.

Procedure Code: The CPT or HCPCS procedure code. Codes that have an end date are not displayed.

Procedure Name: A short description of the procedure code.

Prior Auth Required:

  • Yes: This code requires submission of a prior authorization form and any required documentation. For more information on prior authorization requirements refer to the Prior Authorization Manual.

Coverage Status:

  • Covered: Code is payable by South Dakota Medicaid when coverage criteria are met.  
  • Not Covered: This procedure code is not a covered service by South Dakota Medicaid.

Service Allowable via Telemedicine:

  • Yes: The service may be performed via telemedicine if requirements met. For more information please refer to the provider manuals.
  • No: the service is not eligible to be performed via telemedicine.

Minimum Age and Maximum Age: The recipient’s age on the date of service must be within this age range. If the recipient is younger than the minimum age on the date of service, the code is not payable. If the recipient is older than the maximum age on the date of service, the code is not payable. Codes with a minimum age of “0” and a maximum age of “0” are limited to recipients younger than 12 months and are payable if the diagnosis description indicates it is appropriate for the recipient.

Fee: The maximum allowable reimbursement rate for the procedure. Providers are required to bill Medicaid at their usual and customary charge.

Fee Effective Date: The date the displayed fee became effective.
