Behavioral Health Provider Accreditation Search

The South Dakota Department of Social Services, Office of Licensing and Accreditation accredits Prevention, Substance Use Disorder (SUD) agencies and Community Mental Health Centers to monitor compliance with Administrative Rules of South Dakota (ARSD). The office conducts biennial or triennial on-site reviews. Following the on-site review, an accreditation report is generated that may require a plan of correction of any issues of non-compliance found during the on-site review. The agency will be required to submit their plan of correction with the agency’s plan of action for each area of non-compliance found. The Office of Licensing and Accreditation recognizes agencies who are accredited by a national accrediting body in accordance with SDCL-34-20A-2.1. These agencies are considered deemed status accreditation, and it will be noted on their certificate.

Document Descriptions:

  • Certificate Issued Date: Date listed reflects the month and year the certificate was issued. The Accreditation Certificate indicates the start and end dates of an agency’s accreditation and the services the agency is accredited to provide.
  • On-Site Review Report: Summary of the biennial or triennial on-site review. On-site Review will be noted as N/A for those agencies who are considered deemed status accreditation.
  • Plan of Correction: A standard form sent to the agency after a visit is completed. This provides notification to the agency outlining any issues of non-compliance, which require the agency to form a plan of correction for each item listed, and a timeframe in which correction is expected. Plan of Correction will be noted as N/A for those agencies who are considered deemed status accreditation.
  • Department of Health Reports: Summary of the results of an announced visit by the Department of Health to ensure compliance of Health and Safety Codes, which occurs annually for inpatient and residential facilities.
