Provider Fee Schedule

Medicaid fee schedules are provided below by provider type. Fee schedules generally include procedure codes, procedure code descriptions, and the maximum allowable reimbursement rate. Providers should bill Medicaid for services at the usual and customary charge that they charge the general public rather than the fee schedule rate. Fee schedules are subject to review and amendment in accordance with Administrative Rule of South Dakota (ARSD).

The rate of payment for “Price By Report” is generally the percent of the usual and customary charge established in ARSD 67:16:02:03.

The Department requires providers to bill claims using authorized modifier codes when applicable to the service provided.
